Just a heads up to all users...
There's a class called dompdf which is located at:
http://www.digitaljunkies.ca/dompdf/usage.php
It is easier, IMHO, than the bundled tcpdf class to use if you need to create docs from scratch and not simply tweak what is offered (that is, it's easier if you already know HTML coding, PHP and Smarty).
You can set up your custom doc as a Smarty template, fetch it, and pass the resultant HTML to dompdf. Adjust the stylesheet using standard css language.

thanks for the note. i know you've already done a lot of work on the pdf generation, so i didn't want to request it directly. but i think people who are starting a form from scratch might find it a little easier to deal with, imho.
